Patient Portal

All new and existing patients can create patient accounts with us through the portal.

In the portal, you can:

• Update your contact information
• Sign consent forms prior to arrival to maximize treatment time
• View scheduled appointments
• Request appointments (Coming soon...)
• View your previous invoices

To access your account:

• If you are an existing patient and we have your email address on file, your account has already been created
• Click the Forgot Password link and use the password that is emailed to you from the portal to access your account
• If you are a new patient or we do not have your email address on file, click "Create an Account"
• Enter your contact information
• You will be sent an activation code via email
• Check your junk mail if you don't see your code
